目的:
本研究では、ベーチェット病(BD)患者における下部尿路症状(LUTS)の有病率を明らかにし、BDの重症度とLUTSの関係を検討することを目的とした。
PURPOSE: In this study, we aimed to determine the prevalence of lower urinary tract symptoms (LUTS) in patients with Behçet disease (BD) and to investigate the relationship between the severity of BD and LUTS.
方法:
2018年1月から12月までの期間に,BDを有する55人の患者を本研究に参加させた.すべての患者は詳細な泌尿器科的評価を受けた。さらに、Behçet Disease Current Activation Form を用いて BD 活動性を調査した。国際前立腺症状スコア(IPSS)、国際失禁相談質問票ショートフォーム、8項目の過活動膀胱質問票、尿流測定、排尿後の残尿量を評価した。
METHODS: Fifty-five patients with BD were included in this study from January to December 2018. All patients received a detailed urological evaluation. Additionally, BD activity was investigated using the Behçet Disease Current Activation Form. The International Prostate Symptom Score (IPSS), International Consultation on Incontinence Questionnaire Short Form, 8-item overactive bladder questionnaire forms, uroflowmetry, and postvoid residual urine volume were evaluated.
結果:
患者の平均年齢は42.6歳であった。BDの平均活動スコアは4.6±3.2、平均IPSSスコアは7.3±7.3であった。性別はBD活動スコアに有意な影響を与えなかった。BD 活動スコアは、IPSS スコア、日中排尿回数、夜尿症、切迫感と有意な相関があった。また,尿失禁の有無や重症度,日常生活における尿失禁の影響とも相関していた。ピークフローレート,排尿量,排尿後の残尿量は BD 活動スコアとは相関しなかった.
RESULTS: The mean age of the patients was 42.6 years. The mean activity score of BD was 4.6±3.2 and the mean IPSS score was 7.3±7.3. Sex had no significant effect on the BD activity score. The BD activity score was significantly correlated with the IPSS score, frequency of daytime urination, nocturia, and urgency. It was also correlated with the presence and the severity of urinary incontinence and the effect of urinary incontinence on daily life. Peak flow rate, voided volume, and the postvoid residual urine volume were not correlated with the BD activity score.
結論:
下部尿路症状はBD患者において高い有病率を示し,BDの重症度と現在の活動性は頻度,夜尿症,切迫感,尿失禁と相関していた.したがって,本疾患の活動性・重症度が高い BD 患者の泌尿器科的評価を総合的に行い,適切な管理を行う必要がある.
CONCLUSION: LUTS were highly prevalent in patients with BD, and the severity and current activity of BD were correlated with frequency, nocturia, urgency, and urinary incontinence. Therefore, a urological evaluation of patients with BD in whom the activity and severity of the disease is elevated should be carried out comprehensively and patients should be managed accordingly.
