目的:
本研究の目的は,一般的に使用されている様々な器具が,in vitroでの歯根端切除術後の歯根表面の形態,閉塞材,および閉塞材と根管壁の界面に与える影響を評価することである.
AIM: The aim of this study was to evaluate the effects that various commonly used instruments have on root surface morphology, the obturating material, and the interface between the obturation and the root canal walls following root-end resection in vitro.
方法論:
完全に形成された頂部を持つ60本のヒトの単根歯を採取し,装飾を施した.根管にインスツルメントを施した後,AH-26をシーラーとして使用した熱可塑性ガッタパーチャで閉塞した.根を無作為に12のグループに分け,8種類の器具構成と,回転方向に対して根面上を移動する2種類の方向を用いて,根尖切除を行った。切除した歯根端をエポキシ樹脂で再現し,走査型電子顕微鏡で観察した。
METHODOLOGY: Sixty human single-rooted teeth with fully formed apices were collected and decoronated. The root canals were instrumented, and then obturated with thermoplasticized gutta-percha using AH-26 as the sealer. The roots were randomly divided into 12 different groups, and apical root-end resections were performed using eight different instrument configurations, and two different directions in which the bur moved across the root face in relation to its direction of rotation. Epoxy resin replicas of the resected root ends were examined using scanning electron microscopy.
結果:
どの器具も,切除された歯根端に,その切削形状を反映した特徴的な表面仕上げを施した。使用したバーのデザインにかかわらず,根面上のガッタパーチャのスミアや破砕は,ハンドピースをバーの回転方向と逆方向に根面上で移動させた場合にのみ生じた。
RESULTS: Each instrument produced a characteristic surface finish on the resected root end that mirrored its cutting profile. Irrespective of the design of the bur used, smearing and shredding of the gutta-percha across the root face occurred only when the handpiece was moved across the root face in reverse direction in relation to the direction of rotation of the bur.
おわりに:
根面充填物の破壊や歪みを最小限に抑え、ガッタパーチャ界面の破砕を防ぐためには、根管を横切るバーの最終パスが、その回転方向に対して正しい方向になるように注意すべきである。
CONCLUSIONS: To ensure minimal disruption and distortion of the root filling and to prevent shredding of the gutta-percha interface, care should be taken to ensure that the final pass of the bur across the root canal is in the correct direction in relation to its direction of rotation.
