[Possibilities and limitations of treatment with clear aligners. An orientation].

この研究の目的は、クリアアライナーシステム、特にインビザライン®を用いた矯正治療の効率と効果を評価することでした。文献のレビューに加えて、4人の矯正歯科医と固定式矯正治療とインビザライン®で治療を受けた9人の患者にインタビューを行いました。文献によると、エクストルージョン、ローテーション、オーバージェットをクリアアライナーで修正することは難しいとされています。最終的に最良の結果を得るためには、しばしば「リファインメント」（治療中の追加介入）が必要となります。インタビューに答えた患者は、クリアアライナーによる日常生活の制限をほとんど感じず、痛みもほとんどありませんでした。治療期間は固定装置を使用した場合と同程度ですが、矯正歯科医の経験、症例の複雑さ、患者さんの協力に左右されます。インビザライン®治療の費用は、固定式装置による治療よりも高くなります。さらに、クリアアライナーの治療計画を立てることは、固定式アプライアンス治療の治療計画よりも矯正医にとって時間がかかることが多いです。

The aim of this research was to evaluate the efficiency and effectiveness of an orthodontic treatment with clear-aligner systems, specifically with Invisalign®. In addition to a review of the literature, 4 orthodontists and 9 patients treated with fixed orthodontic therapy and Invisalign® were interviewed. The literature showed that it is difficult to correct an extrusion, rotation or overjet with clear aligners. Often 'refinement' (additional intervention during treatment) is necessary to achieve the best possible end result. The patients interviewed experienced few limitations in their daily lives caused by the clear aligners and hardly any pain. Treatment duration was comparable to that with fixed appliances; it depended on the experience of the orthodontist, the complexity of the case and cooperation of the patient. The cost of an Invisalign® treatment is higher than that of fixed-appliance therapy. Additionally, making a plan for clear-aligner treatment often takes more time for the orthodontist than planning treatment for fixed-appliance therapy.

Samenvatting。この研究の目的は、インビザライン®を用いたクリアアライナー治療の効果と効率を評価することです。文献調査では、4人の歯科医師と、他の装置とインビザライン®を使用して治療を受けた9人の患者にインタビューを行いました。最もよく知られている文献によると、クリアアライナーを用いてエクストルージョン、ローテーション、オーバージェットを矯正することは、非常に有効であるとされています。しかし、より良い結果を得るためには、"リファインメント"（歯列矯正後の歯列矯正）が重要です。ほとんどの患者さんは、日常生活や日常生活の中で、クリアアライナーを使ってわずかな治療を行っています。矯正歯科医の診察、ケースの複雑さ、患者さんの協力などを考慮すると、治療費は他の装置を使った場合と比べてわずかしかかかりませんでした。インビザライン矯正の費用は、他の矯正装置を使用した矯正よりも高くなります。また、矯正歯科医は、クリアアライナーを使った治療を計画するために、より多くの時間を必要とします。

Samenvatting. Het doel van dit onderzoek was het evalueren van de effectiviteit en de efficiëntie van een orthodontische behandeling met het clear-alignersysteem volgens Invisalign®. Naast een literatuuronderzoek werden 4 orthodontisten en 9 patiënten, die waren behandeld met vaste apparatuur en Invisalign®, geïnterviewd. Uit de bestudeerde literatuur bleek dat het corrigeren van extrusie, rotaties en overjet met clear aligners moeilijk is uit te voeren. Vaak is ‘refinement’ (ingrijpen tijdens de behandeling) nodig om een zo goed mogelijk eindresultaat te behalen. De ondervraagde patiënten ervoeren weinig beperkingen van de clear aligners in hun dagelijkse leven en ook nauwelijks pijn. De duur van behandelingen waren net zo lang als die met vaste apparatuur: afhankelijk van de ervaring van de orthodontist, de complexiteit van de casus en de coöperatie van de patiënt. De kosten van een Invisalign®-behandeling zijn hoger dan die van een behandeling met vaste apparatuur. Daarnaast kost het een orthodontist meer tijd om een behandeling met clear aligners te plannen.
