歯周病学における抗菌剤の使用

機械的なクリーニングでは、歯の表面、例えばファーケーション部位やポケットの底などの細菌をすべて除去することは不可能です。抗菌剤の使用による非外科的歯周治療のサポートは、したがって、より良い治療結果を達成するための明白なアプローチです。アモキシシリンとメトロニダゾールの組み合わせは、歯周治療のサポートとして徹底的に研究されており、効果的な治療のための歯科医療の専門家が持っているツールの中で場所に値する。しかし、現在のところ、どのような臨床状況で抗生物質を処方しても問題ないという厳密な適応はありません。医療従事者による抗菌薬の責任ある使用（antimicrobial stewardship）を考慮すると、抗生物質による歯周治療をサポートするかどうかは、処方を許可された者の判断に委ねられるべきである。臨床治療の効果を高めるために、歯周治療中およびその後の2週間（6～8週間）にクロルヘキシジンによる1日2回の追加洗浄を検討することができます。

With mechanical cleaning, it is never possible to remove all bacteria from tooth surfaces, in, for example, furcation sites and at the bottom of the pocket. Supporting non-surgical periodontal treatment with the use of antimicrobial agents is, therefore, an obvious approach in order to achieve a better treatment outcome.. The combination of amoxicillin and metronidazole has been thoroughly investigated as a support in periodontal treatment and deserves a place among the tools the dental care professional has for effective treatment. However, there is at present no strict indication in which clinical situations antibiotics can be successfully prescribed. Considering responsible use of antimicrobials by healthcare professionals (antimicrobial stewardship) the decision to support periodontal treatment with antibiotics should be in the hands of those authorised to provide a prescription. Additional rinsing with chlorhexidine twice daily during periodontal treatment and the subsequent two weeks (6-8 weeks) can be considered in order to enhance the effect of clinical treatment.

機械的に再洗浄することで、歯周病菌が毛皮やポケットの中に入ってしまうことはありません。慢性歯周病の治療では、抗生物質を使用した小児外科的治療を行うことで、手指の痛みを軽減することができますが、それ以上の治療効果はありません。アモキシシリンとメトロニダゾールの組み合わせは、歯列矯正の治療のために必要なものであり、中途半端な治療では、患者の負担を軽減することができます。この表示は、他の人が何もしていない場合には、その人が何もしていないことを示しています。さらに、クロルヘキシジンを2日間使用すると、歯列矯正の効果が得られ、2週間後（6～8週間後）には、臨床的にも追加の効果が得られることがあります。

Met mechanische reiniging lukt het nooit om alle bacteriën van het tandoppervlak te verwijderen, zoals in furcaties en op de pocketbodem. Daarom kan ondersteuning van de niet-chirurgische parodontale behandeling met antibiotica een voor de hand liggende benadering zijn om een beter behandelresultaat te krijgen. De combinatie van amoxicilline en metronidazol is als ondersteuning van de parodontale behandeling uitgebreid onderzocht en verdient zeker een plaats in de rij middelen die tandheelkundige zorgverleners ter beschikking staan. De indicatie voor al dan niet voorschrijven is niet klip en klaar en derhalve dient de regie hiervoor in het kader van verantwoord antibioticagebruik (antimicrobial stewardship) in handen te zijn van degene die de receptuurbevoegdheid draagt. Additioneel 2 maal daags spoelen met chloorhexidine gedurende een parodontale behandeling en de 2 weken erna (6-8 weken) is te overwegen om klinisch nog een extra effect te bereiken.
