はじめに:

内皮機能障害は、細胞接着分子の増加、白血球のダイアペシス、血管平滑筋緊張、血管壁の過剰な透過性、およびプロコアグラント活性の増加をもたらす。

INTRODUCTION: Endothelial dysfunction leads to an increase in cellular adhesion molecules, leukocyte diapedesis, vascular smooth muscle tone, excessive permeability of vascular walls, and increased procoagulant activity.

目的:

我々は、炎症性腸疾患（IBD）患者において、いくつかの内皮および血小板活性化マーカーの血清レベルが疾患活動性と相関しているかどうかを調査した。

OBJECTIVES: We investigated whether serum levels of several endothelial and platelet activation markers correlated with disease activity in patients with inflammatory bowel disease (IBD).

患者および方法:

本研究では、潰瘍性大腸炎（UC）患者56人、クローン病（CD）患者66人、健常対照40人を対象とした。全血球数、フィブリノーゲン、C反応性蛋白、アルブミン、インターロイキン-6、腫瘍壊死因子α、E-セレクチン、P-セレクチン、単球化学吸引性蛋白1（MCP-1）、可溶性CD40リガンド（sCD40L）、微小粒子（MPs）レベルを測定した。

PATIENTS AND METHODS: This study included 56 patients with ulcerative colitis (UC), 66 with Crohn's disease (CD), and 40 healthy controls. We measured the complete blood cell count, fibrinogen, C-reactive protein, albumin, interleukin-6, tumour necrosis factor α, E-selectin, P-selectin, monocyte chemoattractant protein 1 (MCP-1), soluble CD40 ligand (sCD40L), and microparticles (MPs) levels.

結果:

E-selectin、P-selectin、MCP-1、sCD40L、MPsの中央値には、活動性・非活動性IBD患者と健常対照群との間に有意な差はなかった。UC群のメイヨースケールで表される臨床疾患活動性は、sCD40L（R = 0.32、P = 0.02）、P-selectin（R = 0.32、P = 0.02）および炎症性マーカーと弱い正の相関を示した。CD群の臨床疾患活動指数は炎症マーカーと正の相関があったが，内皮活性マーカーとは正の相関はなかった．

RESULTS: There were no significant differences in the median levels of E-selectin, P-selectin, MCP-1, sCD40L and MPs between patients with active and inactive IBD and healthy controls. The clinical disease activity expressed by Mayo scale in the UC group was positively weakly correlated with sCD40L (R = 0.32, P = 0.02), P-selectin (R = 0.32, P = 0.02) and inflammatory markers. The clinical disease activity index in the CD group was positively correlated with markers of inflammation but not with markers of endothelial activity.

結論:

E-selectin、P-selectin、sCD40L、MCP-1、MPsのレベルは、炎症性腸疾患の活動性が異なる患者間で有意な差はない。しかし、観察された相関関係や統計学的境界に近い結果もあることから、より多くの患者を対象とした研究が必要であると考えられる。

CONCLUSIONS: E-selectin, P-selectin, sCD40L, MCP-1 and MPs levels do not differ significantly between patients with varying activities of inflammatory bowel disease. However, due to the observed correlations, and some results nearing the statistical boundary, it would be necessary to conduct studies on a larger group of patients to verify the observations.
