播種性は、進行性ヒト免疫不全ウイルス（HIV）感染症で最も一般的に報告されているまれな日和見感染症である。重度の免疫再構成炎症性症候群（IRIS）の発症により治療がさらに複雑化する可能性がある。我々は、急性腎不全を引き起こす肉芽腫性間質性腎炎の形でIRISを合併した、進行性HIVの設定で播種性多剤耐性を示した最初の症例を提示する。この症例は、治療の遅れは生命を脅かす可能性があるため、IRISの稀な合併症を認識することの重要性を強調している。

Disseminated is a rare opportunistic infection reported most commonly in advanced human immunodeficiency virus (HIV) infection. Treatment can be further complicated by the occurrence of severe immune reconstitution inflammatory syndrome (IRIS). We present the first case of disseminated multi-drug-resistant in the setting of advanced HIV, complicated by IRIS in the form of granulomatous interstitial nephritis causing acute renal failure. This case highlights the importance of recognizing rare complications of IRIS, as delays in therapy can be life threatening.
