序論:

近視性黄斑孔網膜剥離（MHRD）を有する眼において、内部限界膜（ILM）剥離およびフラップ除去を伴うパープラナ硝子体手術（PPV）後の形態学的および視覚的転帰を、フラップ挿入による転帰と比較すること。

INTRODUCTION: To compare the morphological and visual outcomes after pars plana vitrectomy (PPV) with internal limiting membrane (ILM) peeling and flap removal to that with flap insertion in eyes with myopic macular hole retinal detachment (MHRD).

方法:

MHRD患者45人の46眼を調査した。18 眼を ILM 剥離とフラップ除去を伴う PPV で治療し，28 眼をフラップ挿入で治療した．両群間でベースライン特性と術後所見を比較した。

METHODS: Forty-six eyes of 45 patients with a MHRD were studied. Eighteen eyes were treated by PPV with ILM peeling and flap removal, and 28 eyes by flap insertion. The baseline characteristics and postoperative findings were compared between the two groups.

結果:

症例の平均年齢は70.0±8.5歳であった。最良修正視力（BCVA）は，フラップ除去群で1.51±0.14 logMAR単位から1.31±0.20 logMAR単位に，フラップ挿入群で1.20±0.10 logMAR単位から0.88±0.10 logMAR単位に改善した（P=0.049）．挿入群（96％）では、除去群（50％、P<0.001）に比べて有意に多くのMHが閉鎖されていた。楕円体帯(EZ)と外部制限膜(ELM)の修復も、フラップ挿入群ではフラップ除去群に比べて良好であった(EZ, 6% vs 14%, P=0.003; ELM, 6% vs 36%, P<0.001)。

RESULTS: The average age of the cases was 70.0±8.5 years. The best-corrected visual acuity (BCVA) was improved from 1.51±0.14 to 1.31±0.20 logMAR units in the flap removed group and from 1.20±0.10 to 0.88±0.10 logMAR units in the flap insertion group (P=0.049). A significantly larger number of MHs was closed in the insertion group (96%) than in the removed group (50%, P<0.001). The restorations of the ellipsoid zone (EZ) and external limiting membrane (ELM) were also better in the flap insertion group than in the flap removed group (EZ, 6% vs 14%, P=0.003; ELM, 6% vs 36%, P<0.001).

DISCUSSION/CONCOLUSION:

フラップを挿入してILMを剥離する手法は、フラップ除去よりもMHの閉鎖に効果的で、BCVAが改善され、網膜外側の微細構造の修復にも優れている。

DISCUSSION/CONCLUSION: The technique of ILM peeling with flap insertion is more effective in closing the MH, results in better BCVA, and better restoration of the outer retinal microstructures than flap removal.
