Representing reality is a tricky business and clinical simulation is often critiqued as being inauthentic or lacking realism. Johnston' et al's paper in this issue of Medical Education warns of larger concerns: clinical practice becomes misrepresented to the point that students learn something altogether different. This threat is illustrated with examples such as the Objective Structured Clinical Examination (OSCE) or the simulation centres with their "disembodied plastic body parts" and their "illusion of chaos".
