慢性好酸球性肺炎再発に対するメポリズマブ単独投与の効果：症例報告

抄録

慢性好酸球性肺炎（CEP）は、肺に好酸球が大量に蓄積することを特徴とするまれな疾患であり、血液好酸球症と関連している可能性がある。ステロイドに対する反応は一般的であり、ステロイドの漸減または中止後に再発することが多い；頻繁に長期の経口コルチコステロイド投与が必要である。我々は、CEPの再発時に、慢性的な全身ステロイド療法やステロイド併用療法を行わずに、メポリズマブ単独で治療に成功した臨床例を報告した。臨床症状、胸部CTスキャン、生物学的マーカーの改善が観察された。この症例は、再発や重篤な合併症が発生した場合に、メポリズマブがCEPの優れた治療法である可能性を確認したものである。抗インターロイキン5療法がこれらの患者の血管炎などの全身疾患を発症するリスクを減少させるかどうかはまだ明らかになっていない。

Chronic eosinophilic pneumonia (CEP) is a rare disorder characterized by the massive accumulation of eosinophils in the lungs and may be associated with blood eosinophilia. The response to steroids is common as well as a relapse after tapering or suspension; frequently long-term oral corticosteroid administration is needed. We described a clinical case successfully treated with mepolizumab alone during a relapse of CEP, instead of chronic systemic steroids or concomitant steroids therapy as it usually occurs. We observed an improvement of clinical symptoms, radiological thoracic CT scan and biological markers. This case confirms that mepolizumab might be an excellent treatment for CEP when relapses or serious complications occur. It is yet to be known if anti-interleukin 5 therapy could reduce the risk to develop systemic disorders as vasculitis in these patients.
