背景:
腎細胞がん(RCC)は、腎尿細管上皮細胞に由来する腫瘍であり、成人の泌尿器系腫瘍の中で2番目に発生率が高い。しかし、ほとんどの場合、RCCの初期または後期には明らかな症状が見られず、予後に大きな影響を与えている。本研究では,血管内皮増殖因子(VEGF)およびエクト-5'-ヌクレオチダーゼ(CD73)の発現を解析し,RCCにおける臨床病理,微小血管密度(MVD)および予後との関連を明らかにすることを目的とした.
Background: Renal cell carcinoma (RCC), a tumor originating from renal tubular epithelial cells, has the second highest incidence of all adult urogenital tumors. However, in most cases, patients show no obvious symptoms in the early or even the late stages of RCC, which seriously impacts the prognosis. This study aimed to analyze the expression of vascular endothelial growth factor (VEGF) and ecto-5'-nucleotidase (CD73) and their relationship with clinical pathology, microvessel density (MVD), and prognosis in RCC.
方法:
2011年10月から2013年10月までの間に当院で根治的腎摘出術を受けたRCC患者76名の臨床データをレトロスペクティブに解析した。術後パラフィン標本を採取し,腫瘍組織中のVEGFとCD73の発現量を検出し,MVDを測定した.RCCにおけるVEGFおよびCD73の発現量と臨床病理、MVD、予後との関係をTで調べた。
Methods: The clinical data of 76 patients with RCC who underwent radical nephrectomy in our hospital between October, 2011 and October, 2013 were retrospectively analyzed. The postoperative paraffin specimens were collected; the expression levels of VEGF and CD73 in the tumor tissues were detected, and the MVD was measured. T the expression of VEGF and CD73 and their relationship with clinical pathology, MVD, and prognosis in RCC.
結果:
グレードG3~G4のRCC患者におけるVEGFおよびCD73の陽性発現率は、グレードG1~G2の患者よりも高かった(P<0.05)。III~IV期のRCC患者の発現率は、I~II期の患者よりも高かった(P<0.05)。リンパ節転移のあるRCC患者の割合は、リンパ節転移のない患者よりも高かった(P<0.05)。VEGFおよびCD73の発現が陽性の患者のMVD数は、発現が陰性の患者よりも高かった(P<0.05)。RCC組織におけるVEGFおよびCD73の発現はMVD数と有意に正の相関を示した(P<0.05)。VEGFおよびCD73の発現が陽性の患者の5年死亡率は、発現が陰性の患者よりも高かった(P<0.05)。
Results: The positive expression rates of VEGF and CD73 in RCC patients with grades G3-G4 were higher than those in patients with grades G1-G2 (P<0.05). The rates in RCC patients with stages III~IV were higher than those in patients with stages I-II (P<0.05). The rates in RCC patients with lymph node metastasis were higher than those in patients without lymph node metastasis (P<0.05). The MVD count of patients with positive expressions of VEGF and CD73 was higher than that of patients with negative expressions (P<0.05). The expressions of VEGF and CD73 in RCC tissues were significantly positively correlated with MVD count (P<0.05). The five-year mortality rate of patients with positive expressions of VEGF and CD73 was higher than that of patients with negative expressions (P<0.05).
結論:
RCC組織におけるVEGFとCD73の発現は、腫瘍悪性度、浸潤、転移の程度を反映することができ、腫瘍組織における微小血管の形成や患者の予後不良と密接に関連しています。
Conclusions: The expressions of VEGF and CD73 in RCC tissues can reflect the degree of tumor malignancy, invasion, and metastasis, and are closely related to the formation of microvessels in tumor tissues and the poor prognosis of patients.
