インビザライン®システムによる上顎・下顎の拡大の有効性と予測可能性

Efficacy and predictability of maxillary and mandibular expansion with the Invisalign® system.

背景:

本研究の目的は、インビザライン®システムを用いた上顎・下顎の矯正的拡大の有効性と予測性を評価することである。

Background: The purpose of this study is to evaluate the efficacy and predictability of upper and lower orthodontic expansion with the Invisalign® system.

材料と方法:

167名の患者の中から、プラスチック矯正（インビザライン®アライナー、SmartTrack®マテリアル）で治療を受けた患者から、上顎64名、下顎51名のアーチを無作為に抽出した。デジタルモデルは、治療開始時（ModT1）と治療終了時（ModT2）、および最終的なClinCheck®（CkT2）で抽出されました。犬歯、小臼歯、大臼歯の幅は、上顎第一大臼歯の傾斜と同様に、両アーチの歯肉と犬歯のレベルで測定された。同様に、第一大臼歯のレベルで計画された拡張に関して、両歯列を軽度、中等度、重度に分けた。

Material and Methods: From a sample of 167 patients; 64 upper and 51 lower arches were randomly selected from patients who had been treated with plastic orthodontics (Invisalign® aligners, SmartTrack® material). Digital models were extracted at the beginning (ModT1) and at the end of treatment (ModT2) as well as the final ClinCheck® (CkT2). The canine, premolar and molar width was measured at the gingival and cuspid level of both arches, as well as the inclination of the upper first molar. Likewise, both arches were divided regarding the planned expansion at the level of the first molar into mild, moderate and severe.

結果:

上下の歯列における拡大の効果は、統計的に有意な差を示した（<0.00005）。予測可能性の測定では、歯冠レベルでは98～100％、歯肉レベルでは85～90％が達成された。膨張の大きさに関する予測性を分析すると、歯肉と犬歯の両方で、上側と下側の中程度の膨張が高い値を示した。

Results: The efficacy of expansion in the upper and lower arches showed a statistically significant difference (<0.00005). During the measurements of predictability, around 98-100% was achieved at the coronal level and between 85-90% at the gingival level. Analyzing predictability regarding to the magnitude of expansion, superior and inferior moderate expansion, both gingival and cuspid, presented higher values.

結論:

インビザラインシステムのアライナー（スマートトラック®素材）は、上下顎ともに予測性の高い拡大運動を可能にするポジティブな選択肢であることがわかりました。その結果、最も予測可能な拡大レベルは中程度であり、上顎アーチよりも下顎アーチの方が歯肉レベルで予測可能であった。予測可能性、有効性、拡大、アライナー、インビザライン®。

Conclusions: The Invisalign® system aligners (SmartTrack® material), proved to be a positive alternative for expansion movement offering high degree of predictability, both in the upper and lower arches. As a result, the most predictable level of expansion was moderate, having being the lower arch more foreseeable at the gingival level than the upper arch.
