激しいブラキシズムの後に発症した歯根膜炎について

Apical periodontitis after intense bruxism.

背景:

ブラキシズムは、咀嚼筋痛、顎関節痛、頭痛、機械的歯牙摩耗、補綴合併症、歯牙亀裂の原因となることが知られています。しかし、ブラキシズムが歯髄を損傷する可能性があることは、あまり知られておらず、文献でも実験的にしか報告されていない。本症例は、ブラキシズムによって歯根端部歯周炎を発症した最初の臨床例として知られている。

BACKGROUND: Bruxism is known to cause masticatory muscle pain, temporomandibular joint pain, headaches, mechanical tooth wear, prosthodontic complications and cracked teeth. Less known to the practitioner, and described only experimentally in literature, is that bruxism can also damage the pulp. To our knowledge, this is the first known clinical case of a patient developing apical periodontitis due to bruxism.

症例提示:

本稿では、28歳の健康な男性患者が、ブラキシズムの激しい局面を経て、歯36と歯46に根管治療を必要とする歯根端歯周炎を発症し、治療が成功した事例を紹介する。診断が不明確であったため、治療が遅れていた。

CASE PRESENTATION: This article presents the case and successful treatment of a 28-year-old healthy male patient with apical periodontitis on teeth 36 and 46 requiring root canal treatment after an intense phase of bruxism. Due to an unclear diagnosis, treatment had been delayed.

結論:

理解しがたい歯の痛みは、ブラキシズムの結果である可能性があります。強いブラキシズムは、歯根端炎を引き起こす可能性があることを、医師は知っておく必要があります。したがって、診断の際には、十分なアナムネーゼを収集し、それを考慮することが重要である。

CONCLUSIONS: Incomprehensible tooth pain can be the result of bruxism. Practitioners need to be informed that intense bruxism can possibly lead to apical periodontitis. It is important, therefore, that a thorough anamnesis is collected and taken into account during diagnostics.
