はじめに:

本研究の目的は、下顎第三大臼歯の埋伏歯除去術における4%アルチカイン、0.5%ブピバカイン、0.5%ロピバカイン（1:200,000アドレナリン併用）の麻酔効果を比較することである。

INTRODUCTION: The aim of this study was to compare the anesthetic efficacy of 4% articaine, 0.5% bupivacaine and 0.5% ropivacaine (with 1:200,000 adrenaline) during surgical removal of impacted mandibular third molars.

材料と方法:

75人の患者を無作為に25人ずつ3群に分けた。研究変数は、麻酔作用の発現、手術時間、麻酔時間、術後鎮痛時間であった。痛みの評価には視覚的アナログスケールを用いた。統計解析の結果、使用した麻酔液の量、麻酔の質、手術の難易度、手術時間に関して、群間に有意差は認められなかった。

MATERIALS AND METHODS: The study included 75 patients randomly divided into three equal groups of 25 patients each. The study variables were: onset of anesthetic action, duration of surgery and anesthesia and postoperative analgesia. A visual analog scale was used to assess pain at different time intervals. Statistical analysis revealed insignificant difference among groups in terms of volume of anesthetic solution used, quality of anesthesia, surgical difficulty and duration of surgery.

結果:

平均麻酔開始時間はアルチカイン（1.14分）がロピバカイン（2.18分）およびブピバカイン（2.33分）より有意に短かった（<0.001）。しかし、麻酔時間および鎮痛時間は、アルチカイン（232.8分、191.4分）に対してブピバカイン（483.6分、464分）、ロピバカイン（426.6分、459分）がそれぞれ有意（<0.001）に長かった。また、術後6時間目の疼痛スコアは3群間で有意であった（<0.01）。

RESULTS: The mean onset time was significantly ( < 0.001) shorter for articaine (1.14 min) than ropivacaine (2.18 min) and bupivacaine (2.33 min). However, the duration of anesthesia as well as analgesia was significantly ( < 0.001) longer for bupivacaine (483.6 min and 464 min) and ropivacaine (426.6 min and 459 min) as compared to articaine (232.8 min and 191.4 min), respectively. Also, on comparing three groups pain scores at 6th postoperative hour were significant ( < 0.01).

結論:

ロピバカインとブピバカインは、手術時間が長くなることが予想される患者に安全に使用できる。

CONCLUSION: Ropivacaine and bupivacaine can be safely used in patients where longer duration of surgery is anticipated.
